Recertification for Hospice Care Hospice is required to review the patient's medical condition on a periodic basis to ensure they are still eligible for hospice services. In most cases there is no change in eligibility. In a few cases the patient's medical condition has stabilized or improved and hospice services must be discontinued. This is most likely to occur for patients with cardiac or pulmonary conditions. If discharge is necessary, a family meeting will be held with the social worker to assist in planning for continued care from other agencies. In all cases the family will be given at least two weeks notice prior to discharge. After discharge some services will continue to be provided through our Family Support program. If the patient's condition worsens, certified hospice services could again be provided. This procedure is necessary to ensure our agency complies with insurance rules and to ensure coverage for the care provided.
