Hospice Social Workers assist patients and their families with the maze of decisions, arrangements and emotional difficulties at the stressful time that accompanies terminal illness.
- They visit patients and their families in their homes, skilled nursing facilities, hospitals, or wherever they are needed.
- They are available to the patient, family and anyone emotionally vested in the patient.
- Hospice Social Workers can help children, family or friends through the sensitive discussion of terminal illness.
- They can be an objective listener when family members have differing ideas about what is right for their loved one.
Our professional staff assist individuals through listening, reflecting and supportive counseling.
- They can help families identify strengths that will unite and empower them to care for their own.
- Social Workers assist family members in developing a long-term plan of care for their loved one at home or a skilled nursing facility.
- They can also provide anticipatory grief counseling that will help to prepare family members for the death of their loved one.
Other services provided by Hospice Social Workers include:
- Answering questions regarding medical bills and expenses
- Assisting with forms, such as Advance Directives, Health Care Proxies or Powers of Attorny
- Referral to programs such as Lifeline, Meals On Wheels, Office For The Aging, CASA, Medicaid, EPIC, HEAP, etc.
- Assisting with funeral planning
